We are dedicated to providing you with high-quality care in a comfortable and welcoming environment. Here’s what you can expect during your first visit and what you should bring to ensure a smooth experience.

1. Intake Form

After booking your appointment through our online booking system or with one of the receptionists, you will receive a booking confirmation from Jane/Pro Health. Along with the booking confirmation, another email will include an intake form for you to complete. During the intake, you will be prompted to put in your insurance and credit card details. This ensures an easy onboarding process so we can get you in right away. 

Uncomfortable with filling out paperwork online? No worries! You can come in 5-10 minutes earlier than your appointment time to give you enough time to complete your paperwork prior to your treatment. 

Please note: If it has been over a year since your previous appointment, you must book an initial assessment/treatment. This is according to health regulations and policies and gives your practitioner enough time to assess if there are any new concerns properly.

2. Welcome & Check-In

Upon your arrival, our front desk will greet you and assist with any other necessary housekeeping. This may include providing written consent for your treatment, such as with chiropractic care.

3. Comprehensive Health Assessment

Your practitioner will review your medical history, discuss any concerns, and conduct a thorough examination to assess your condition.

4. Personalized Treatment Plan & First Treatment

Based on your assessment, the practitioner will determine the appropriate diagnosis and treatment plan, as well as provide the first treatment.

5. Follow-up care

Your provider will discuss the next steps, including follow-up appointments and at-home care tips to support your progress.
